Repeat Prescriptions

medication4These are generally issued for patients on long- term medication and at the doctor’s discretion. To request a repeat medication:

  • Tick clearly the item/s you require on the computerised repeat request form attached to your last prescription.
  • Completed forms can be placed in the box on reception, and when the surgery is closed placed in the staff entrance post box.
  • Collect your prescriptions from reception after 72hrs and after 4pm
  • Please provide a stamped addressed envelope if you would like your prescription sent back to you.
  • Some local pharmacists offer a repeat prescription service, and also may be able to deliver requested medicines.
  • If you nominate a Pharmacy to collect/request your prescription please notify the surgery so we can update your records.

Help us to maintain an efficient and safe service – so please:

  • Always use the computerised slip – the reception staff can reprint if lost.
  • Note we do NOT accept requests by phone as mistakes can occur.
  • Do NOT ask for medicines which are not on your repeat list. For this you will need to book an appointment with one of our doctors, and any further medicine may be subsequently put on to repeat.
  • Do NOT ask reception for repeat medications as ‘an emergency’. There are very few situations where this is genuinely the case and, you may be asked to book an appointment to see a doctor. Leave plenty of time before weekends, bank holidays and vacations.

Electronic Prescription Service   Is an NHS service. Its gives you the chance to change how your GP sends your prescription to the place you choose to get your medicines or appliances from. Deben Road Surgery is due to go live on Tuesday the 18th of October 2016. For more information please speak to the Receptionist or your local Pharmacy.

Warfarin Patients

Please bring in a copy of your result notification slip each time you request Warfarin so we are sure of the latest INR result and, your current dose before we prescribe.
